Medical Marijuana Card FAQ – Do I Qualify?

Q: What are the Medical Marijuana Card application requirements?

A: To apply for a medical marijuana card, you must be:

  • A Colorado resident with a valid Social Security number.
  • Receiving treatment for a Qualifying Medical Condition.
  • Examined by a doctor with whom you have a bona fide physician-patient relationship.
Q: Which medical conditions are Qualifying Medical Conditions?

A: Medical marijuana card may be recommended for the following debilitating medical conditions or a debilitating medical condition that produces one of the following:

Cancer Glaucoma
HIV or AIDS Cachexia
Persistent muscle spasms Seizures
Severe nausea Severe pain
Q: Are there regulatory requirements for authorization?

A: Yes. You must be able to sign this statement:

  • I have never been convicted of a criminal offense involving controlled substances, and
  • I have never been sentenced or ordered by a court to drug or substance abuse treatment, and
  • I have never been sentenced to the division of youth corrections.
Q: Does Dr. Ament’s office have any additional requirements?

A: Yes. Here they are:

  • Adults Only, age 21 or older.
  • A history of substance misuse or psychiatric hospitalization requires additional evaluation, and in some cases may, at the discretion of the physician, disqualify you from receiving a physician certification from this office.
Q: What must I bring to my Medical Marijuana Evaluation appointment?

A: Please arrive prepared with the following:

  • A Colorado driver’s license or Colorado state ID
  • The name and contact information of a Primary Care Physician or Provider whom you have seen within the last 12 months (examples include: Family Medicine, Internist, Osteopathic Physician, Physician’s Assistant, Nurse Practitioner, ObGyn, etc.). If you do not have a Primary Care Physician or Provider, you may still qualify if you have seen a different type of Physician or Provider for your Qualifying Medical Condition within the last 12 months. If you have seen no medical provider in the last 12 months, please establish care with a Primary Care Physician first, in order to qualify.
  • For Adults under the age of 31, supporting documentation from treating physicians is required. Please bring Medical Records and supporting documentation of the Qualifying Medical Condition. These must be from a Medical Doctor (MD), Doctor of Osteopathy (DO), Nurse Practitioner (NP), Physician’s Assistant (PA), podiatrist (DPM), Naturopathic Doctor (ND), or optometrist (OD). Records must include visits within the last three years.
